工伤保险办事指南

一、事项名称

 

工伤保险待遇申报
二、设定依据

 

参保职工发生工伤事故后,根据工伤保险政策法规享受相应的工伤待遇。
三、办理条件

 

职工参加工伤保险后发生的工伤事故,且由人社局鉴定为工伤才能进行工伤待遇申报。
四、办理材料

 

1、工伤门诊、住院药费申报

 

、工伤认定书原件;、已备案的工伤事故报告;、门诊病历及处方;、住院病历及费用清单;、原始发票;、单位银行帐号(户名、帐号、开户行、加盖公章)

 

备注:门诊病历与发日期一致,门诊病历必须为原件,书写规范且有医生签字

 

2、工伤旧伤复发(后续治疗)待遇申报

 

、工伤(职业病)人员旧伤复发认定结论通知书(由人社局鉴定确认)、工伤事故认定书复印件;

、工伤旧伤复发住院(转)审批表;

、门诊、住院病历及费用清单;

、住院费用原始发票

3、工伤保险一次性待遇申报

 

、职工劳动能力鉴定结论书 ;、工伤事故认定书复印件、单位银行帐号(户名、帐号、开户行、加盖公章);、解除劳动关系协议书备注:如未解决劳动关系,则不可申报一次性医疗补助金

 

4、工伤保险工亡待遇申报

 

、工伤工亡认定决定书;、供养亲属户口及身份证复印件;、火化证明

五、办理地点

 

鄂州市医疗保险局厅
六、办理时间及时限

 

工伤认定鉴定后三个月内前来办理,审批流程约 30 个工日。
七、办理流程

 

参保人员发生工伤事故后、24 小时内需向市医保局电报告工伤事故情况,3 个工作日内将工伤事故以书面形式报市医保局工伤科备案。职工工伤后必须由市人社局进行工伤认定,认定为工伤才能行工伤待遇申报。

九、注意事项

 

工伤保险报销费用及待遇全部通过对公单位银行账号转账,如有特殊情况单位无法提供对公银行账号的,需由单位出示委托书,并注明转入账号用途。
